feat(core): import_lastpass — group, favorite, notes

Map LastPass grouping/fav/extra columns to relicario item metadata.
Grouping becomes item.group, fav="1" sets item.favorite, extra becomes item.notes.
Multi-line extra via CSV quoting round-trips correctly.
