Fixture CSV exercises 11 rows: standard login, login + TOTP,
SecureNote (plain + structured), unicode title, bad URL,
malformed rows. Tests verify item count, single git commit,
warning surface area, exit code, and ID uniqueness across
back-to-back imports.
